Tokushukai Information System Inc., (hereinafter referred to as “Company”) has a philosophy of “Aspire for the society of receiving the best available medical care wherever and whoever in good conscience with utilizing the information system”. The Company has been involved in the system development, introduction and operation.

In order to realize the above philosophy, we will handle the personal information of patients, group facility staff, and employees appropriately and rigorously.

Company, therefore, as a specific effort, set the Privacy Information Protection Policy and structure Privacy Information Protection Management System. In accordance with this system, we shall make our employee to recognize the importance of privacy information protection and utilize and manage privacy information adequately.

1. Treatment of Privacy Information

   Company limit the utilization of privacy information within the range of Company’s operation and its related administration and management. Company treat privacy information within this range. Also, except for below mentioned case, Company shall not use privacy information outside of this limitation, in accordance with the set regulation, without having prior written approval/agreement by the principal person.

   • When disclosure is ordered by law or mandatory requested by the governmental organization (Referral from Police and/or Prosecutor organization)
   • When it is necessary for the person to protect one’s life and physical body, as well as when it is difficult to get the consent of the said person.
   • When the Government organization assumes the clerical work stipulated in law or regulation, and it is threatened that the prosecution of the said work becomes difficult or is posed a problem by having the person’s consent.
2. About the compliance of the law

   Company respects the laws and regulations, guideline or other rules set force by Japanese Government in regard to the treatment of Privacy Information Protection.

3. Safety Measures for Privacy Information (Prevention and correction for the Leakage, Loss or prejudice)

   Company execute the best preventive measures against the leakage, loss or prejudice of privacy information treated by the Company. Company assures the safeness and accuracy of privacy information, and even when unexpected incidents occur, Company shall execute prompt countermeasures.
